Summer 2026 Health Form is Here!

The following information was sent to Summer 2026 Trip Leaders by Camps. If you are taking kids to camp in any capacity this summer, regardless of being the official Trip Leader, it would benefit you to be aware of this new process.

Area Admins – I would highly recommend you read through this information and familiarize yourself with the new online health form process.


Before Using the Health Form:

Please review the following steps before sending the health and consent forms to parents/guardians:

  1. Have kids register for camp – Nothing about the new form changes how kids sign up or pay for camp. Camp registration can still be completed as usual.
  2. Plan Your Trip – If your trip includes any pre- or post-camp activities, be sure to add an additional activity to your campaign in YL Connect Camp. By designating these additional activities, the system will include them in the Health and Consent form, and parents/guardians will only have to sign one consent form that covers both camp and any additional activities.
    • Additional activities should be added before individualized Health Form inks are sent. Once forms are sent, changes will require parents to review and re-acknowledge information.

  3. Confirm involvement roles on the Arrival Form – YL Connect Camp will NOT
    allow you to send an individualized health and consent form link to
    parents/guardians if a participant’s involvement role is listed as “participant.”

    Please double-check that involvement roles are accurate, including:
  4. Send individualized Health Form links through YL Connect Camp –
    • NEW: there are no registrant IDs or access codes! Parents/guardians will verify the participant’s last name and date of birth to begin the form.

Need a paper form?

As a reminder, paper forms are now campaign-specific and can be found on your arrival form. Any old versions of the 6007 form will not be accepted at camp this summer. If a paper form is needed, be sure to use the correct, current version for your trip.

Lake Champion & Saranac Specific Information

Immunization Documentation Upload

For Campers/Guests under the age of 18, participants are required to submit immunization records/documentation; parents or guardians will be prompted to provide this information while completing the form.

During the initial form completion, parents/guardians may upload:

  1. A copy of the state-certified immunization record, OR
  2. A valid exemption letter, if applicable

If documentation is uploaded during the initial form completion, no further action is needed.

Parents/guardians may also choose to upload immunization documentation at a later date. In this case, the confirmation email will include a participant-specific upload link that allows documentation to be uploaded directly into YL Connect Camp.

If a parent/guardian cannot locate the upload link, the local area or camp can resend it from YL Connect Camp. The Connections Team can also assist in providing the upload link if needed.

Medication Rider

Participants who take medications, they will be required to complete a medication rider signed by their healthcare provider. The process is now handled through a participant-specific form that populates during the completion of the health form and upload link.

After a parent/guardian completes the form, the confirmation email will include:

  • A link to the participant’s specific Medication Rider form, which can be printed and taken to the prescribing physician for signature.
  • A secure upload link that allows the completed, signed form to be uploaded directly into YL Connect Camp.

Once uploaded, the medication rider is automatically attached to the participant’s record in YL Connect Camp.

If a parent/guardian cannot locate the participant-specific Medication Rider form or the upload link, the local area or camp can resend the link from YL Connect Camp. The Connections Team can also assist in providing either link if needed.
